# Monitoring reports

Record the results of monitoring and link them to the control measures and topics they concern.

A monitoring report records what a monitoring activity found and what follows from it. Uploading reports to Ruler lets you:

* Record the results of monitoring activities
* Determine follow-up actions and track their progress
* Link the results to the control measure that was tested and to the topic from the Legal Framework it covers

## The reports list

The **Monitoring reports** page lists each report with the number of control measures it is linked to and its date of creation. Use the search bar to find a report.

## Adding a report

Click **+ New monitoring report** and fill in:

* **Title**
* **File**: the report itself. This is required, up to 30 MB.
* **Date of creation**
* **Topics**: for each department, the topics from the Legal Framework the report covers.

A report linked to a topic can then be used in the topic's design and effectiveness assessment and in risk assessments. That is what turns a report into evidence: anyone assessing later can see what was tested, when, and with what result.
